OAKLAND, Calif. , Sept. 26, 2024 /PRNewswire/ — Alameda Health System (AHS) has received a new grant to help physicians provide in-home visits to patients.
House calls are a model of care most often associated with old-fashioned or luxury medicine. However, the benefits of house calls may have the greatest impact when applied to vulnerable populations who are disproportionately affected by social determinants of health that negatively impact well-being.

Some AHS patients do not have reliable transportation to and from their doctor’s appointments. This is just one of the structural barriers to equal access to health care. Home visiting programs focus on removing that barrier and addressing the social determinants of health that impact AHS patients.
For example, if a patient is repeatedly admitted to the emergency room, a doctor may use house calls to identify the patient’s underlying health problems or socio-economic factors that prevent access to preventive, regular medical appointments. We try to understand and deal with it.
