Health department temporarily closes Springfield Chinese restaurant

A Springfield restaurant closed temporarily last week for violating its risk control plan, according to a Sept. 24 report from a Springfield-Greene County Health Department’s food inspector.

Triple Eights, 1701 S. Kansas Expressway, closed for 24 hours after a food inspector observed two containers of hot and sour soup being stored at room temperature since the day before and three packages of crab sticks that had been on the counter since that morning, according to the report. This instance was a violation of the risk control plan, which are put in place by inspectors after an establishment has three of the same priority violations in a row.

The Chinese restaurant reopened Sept. 25 after another inspection, according to a report.

Issues found during inspection fall into either priority or non-priority violations. Priority violations impact the safety of the food, such as cross-contamination between raw and ready-to-eat food, improper food temperature and poor personal hygiene and employee health. Multiple priority violations can lead to an establishment being shut down. Non-priority violations alone do not directly affect food safety, such as dirty floors, sticky tabletops or outside trash cans not being covered.

Food inspections take place one to three times a year, depending on the type of food served, the population served, difficulty of food preparation and past history. Restaurants preparing food from raw ingredients are inspected more often “than convenience stores that serve only non-potentially hazardous foods, such as popcorn and soda,” according to the Springfield-Greene County Health Department.

Each report identifies what was happening at the establishment at that time. Consumers who want to know whether a violation is a one-time thing or a pattern are encouraged to refer to previous reports.

Here are food inspections from the past week:

Arris’ Pizza, 1332 E. Republic Road, Springfield. One priority violation: Observed meatballs at 108 degrees Fahrenheit and ground beef at 128 degrees Fahrenheit in the middle of the steam table, which was corrected by notifying the manager who voluntarily discarded the meatballs and ground beef and adjusted the steam table. One non-priority violation: Observed multiple ceiling tiles are missing on the ceiling of hot water heater room.

Aviary by Lavender Falls, 2144 E. Republic Road, Springfield. Sept. 30 routine inspection – Result: permitted with re-inspection. One priority violation: Observed chicken at 44 degrees and chicken salad at 44 degrees in walk-in cooler, which were relocated to back walk-in cooler. No non-priority violations.

Aviary by Lavender Falls, 2144 E. Republic Road, Springfield. Oct. 10 re-inspection – Result: active. At time of re-inspection, walk-in cooler was properly holding temperature.

Braum’s Ice Cream No. 273, 1401 W. Sunshine St,. Springfield. Sept. 25 routine inspection – Result: active. Two priority violations: Observed bag of rotten potatoes in grocery area, which was corrected by staff voluntarily discarding it; observed lower cooler by grill holding sliced tomatoes at 53 degrees and cream cheese at 43 degrees, open face cooler holding salad prep kit at 45 degrees, cheese block at 46 degrees, cookie dough at 44 degrees and hard-boiled egg at 44 degrees, re-inspection date scheduled. Six non-priority violations: Observed grill brush being stored in hand sink, which was corrected on site; observed cartons of raw shell eggs stored above strawberries, which was corrected on site; observed fan cover with debris on walk-in cooler; observed several dishes not properly air-dried prior to being stacked; observed floors not clean behind shelving and equipment; observed sliced tomatoes at 53 degrees and cut lettuce at 63 degrees in prep cooler without time as control marked, which was corrected on site and education given.

Braun’s Ice Cream No. 275, 4125 S. National Ave., Springfield. Sept. 30 routine inspection – Result: active. No priority violations. One non-priority violation: Observed mold development in walk-in cooler on seams of doors and hard to reach areas and top portion of outer ice machine needs to be cleaned.

Brockrito, 38 S. Main St., Fair Grove. Sept. 28 routine inspection – Result: active. No violations noted.

CoxHealth – North, 1423 N. Jefferson Ave., Springfield. Oct. 1 routine inspection – Result: active. One priority violation: Observed shelled eggs stored over ready-to-eat cottage cheese in walk-in cooler, which was corrected on site. No non-priority violations.

Dollar General Corporation, Springfield. Sept. 27 routine inspection – Result: active. No violations noted.

El Maguey, 631 N. Glenstone Ave., Springfield. Sept. 26 re-inspection – Result: active. Previous cold-holding violation corrected by retiring and removing old unit from facility.

Hong Kong Inn, 1421 W. Kearney St., Springfield. Sept. 26 routine inspection – Result: active. No priority violations. One non-priority violation: Observed wiping cloths stored in a chlorine sanitizing solution at 10 ppm, which was corrected by staff.

Hong Yen, 2922 S. Campbell Ave., Springfield. Sept. 30 routine inspection – Result: permitted with re-inspection. Two priority violations: Observed beef at 44 degrees and sprouts at 44 degrees on make table, items were relocated to reach-in cooler and service call made for make table; observed knives being stored unclean on magnetic strip holder in kitchen, which was corrected by removing knives from strip for cleaning. One non-priority violation: Observed outside of rice cooker and microwave have excessive food debris buildup.

Hy-Vee Kitchen, 2150 E. Sunshine St., Springfield. Sept. 30 routine inspection – Result: active. No priority violations. One non-priority violation: Observed rice cooked today cooling with the lid on the container, which was corrected on site by removing lid.

Ichiban Buffet, 625 N. Glenstone Ave., Springfield. Oct. 1 routine inspection. One priority violation: Observed raw tilapia stored over ready-to-eat crab salad, which was corrected on site. One non-priority violation: Observed food stored on ground in walk-in cooler and walk-in freezer, which was corrected on site.

Kickapoo High School Football Concession, 3710 S. Jefferson Ave., Springfield. Sept. 26 routine inspection – Result: active. No violations cited.

Nicola’s Ristorante, 3631 E. Sunshine St., Springfield. Sept. 26 routine inspection – Result: active. One priority violation: Observed shell eggs over box of lemons in walk-in cooler, which was corrected on site. No non-priority violations.

Ohana Japanese Steakhouse, 310 W. Battlefield Road, Springfield. Sept. 26 routine inspection – Result: active. One priority violation: Observed veggie slicer stored dirty with food debris, which was corrected when manager said it was no longer used by establishment and said they would remove it. One non-priority violation: Observed bags of shrimp set in bucket to thaw on counter without refrigeration, shrimp still visibly frozen so violation was corrected by manager placing shrimp into walk-in cooler to thaw.

Two priority violations: Observed multiple items over 41 degrees in large prep cooler by oven, which is a repeat violation and was corrected on site by voluntarily discarding items; observed sliced lunch meats pulled from freezer did not have pull dates on them, which was corrected by discarding items. Four non-priority violations: Observed prep cooler by oven unable to maintain food temperatures below 41 degrees; observed no soap at hand-washing sink in prep kitchen; observed no paper towels at hand-washing sink in kitchen; observed no thermometers on site to check food temperatures.

Scramblers, 1131 E. Republic Road, Springfield. Sept. 26 re-inspection – Result: active. Re-inspection concluded, adviser to continue to monitor lower grill cold holder. No violations noted.

Sequiota School, 3414 S. Mentor Ave., Springfield. Sept. 27 routine inspection – Result: active. No violations cited at time of inspection.

Skully’s, 1427 E. Cherry St., Springfield. Sept. 27 routine inspection – Result: active. One priority violation: Observed multiple sauces such as chipotle aioli and other mayo-based sauces were past expiration date, which was corrected on site by discarding all expired potentially hazardous foods past their expiration dates in cooler. One non-priority violation: Observed back door propped open to prevent self-closing function from operation, which was corrected on site and education provided.

Springfield Southside Seniors, 2215 S. Fremont Ave., Springfield. Sept. 27 routine inspection – Result: active. One priority violation: Observed bagged vegetables in walk-in freezer with ice crystals on bags from walk-in cooler refrigeration lines, which was corrected by discarding vegetables. One non-priority violation: Observed food debris on the non-food contact surface spindle of floor mixer.

The Big Biscuit, 2920 S. Glenstone Ave., Springfield. Sept. 26 routine inspection – Result: permitted with re-inspection. One priority violation: Observed ham at 45 degrees, turkey at 44 degrees, sausage at 44 degrees and tomatoes at 43 degrees in the omelet prep cooler, items were placed on time control and ice baths and re-inspection will occur. No non-priority violations.

One non-priority violation: Observed noodles cooled in covered container in prep cooler, which was corrected by moving noodles to walk-in cooler and uncovering them to finish cooling.
